Any waiver approved using the 1995 Version of the waiver application or any other version must wait until renewal to utilize the web-based application General: Existing approved and eligible waivers must be entered into the web-based system if states would like to use the web-based application to amend waivers or to facilitate future renewals of waivers. Guidance: To enter existing waivers into the web-based system, follow the directions below: 1. Prior to entering data on the web, the State must contact the Regional Office (RO) analyst. Indicate interest in migrating the currently approved Word Version into the web-based format. The RO will ensure that the Central Office (CO) is informed of the intended migration. Obtain the most current version of the approved waiver that includes all approved amendments. It is easier to enter the waiver information with the amendments included than it is to enter each previously-approved amendment individually. The information on the web-based application must match the approved waiver document. Please note that if a service has been added or deleted as a result of an amendment and Appendix J is different from one year to the next, the State should enter 0 for the values in the demonstration of Factor D for the years in which the service was not available through that waiver. Determine which version of the Word application was used to submit the waiver for approval. Only applications approved using Word version 3.1 -3.4 may be entered. The web-based system corresponds with the sections and questions contained in Word Version 3.4. If Word Version 3.4 was not used to initially submit the waiver or renewal, the states should carefully compare their applications with Word Version 3.4. All data that is required in Word Version 3.4 of the application will be required by the web application. States should compare these documents prior to beginning any work on the web-based application. The states should discuss the results of these comparisons with CMS prior to entering any information on the web. CMS will provide suggestions and guidance where there are differences between the approved Word Version of the waiver and the web-based version of the application. CMS will provide guidance on the inclusion or reference of documents that are not transferable to the web-based application for previously approved waivers. The web system will apply all validation edits. These rules are built into the web program, and apply even though the waiver has already been approved in the Word form. In order to use the web waiver for amendments or future renewals on the web, all required fields must be complete. An existing waiver that was a new waiver when it was approved should be entered into the web-based system as a new waiver. An existing waiver that has an approved renewal should be entered as a renewal into the web-based system. When entering data in the web-based application, enter the correct effective date for the waiver even though this date is in the past (the State will enter the approved effective date in the proposed effective date field). This effective date is the date of the most recent RENEWAL or the official approval date for a new waiver. For renewal or initial waiver with previously approved amendments, use the effective date of the renewal or the date the waiver was approved for a new/initial waiver (instead of the effective dates of any subsequently approved amendments). Please note that if the State had submitted and received approval for an amendment changing the effective date of the waiver, the State should indicate the newly approved waiver effective date. For waivers being loaded into the system that have been amended since the approved renewal/amendment using version 3.1-3.4, the State can indicate in the Brief Waiver Description section the amendment approval dates and the nature of the amendments. When entering data into the web-based application for Appendix J, enter information for all years of the waiver regardless of the current life-cycle year of the waiver. For example, for a renewal currently in waiver year three, data will still need to be entered for waiver years one and two. If the State has amended certain years of this data, the waiver in the web-based application should reflect the most recently approved information for that waiver. States experiencing difficulties with adapting the approved versions of existing waivers with the web version should discuss the issues with their CMS analysts. Any work-around or accommodation needed to allow submission using the web-based application (and passage of relevant edit checks) must be explicitly approved by CMS.
