Web-Based 1915(c) HCBS Waiver Application Log-In/Log-Out Information Q. Do I have to log out of the system when I am finished working on the waiver application and want to exit the system? A. It is best to select the Logout link when you're finished with the application site and then close your browser. By doing so, you prevent others from potentially gaining access to your application in your absence. Q. How do I change my password? A. Enter your User Name and Password in the Login screen. Do not log in! Select Change Password at the top of the screen. The Change Password screen will display with your User Name. Enter your new password and re-enter it to confirm. Select Change to create the new password or Cancel to void this transaction. Q. What do I do when I receive an invalid log-in and I cant remember my password? A. If you receive an invalid log-in and cant remember your password, contact your State Sys/Op Manager or a staff member at Thomson Reuters to reset your password to the default password. Entering Information into the Waiver Application Q. Can I copy information from a Microsoft Word document and paste it into the web application? A. Yes. The text boxes will accept pasting of Word document content as long as it is devoid of Word unique characters such as smart quotes. Be advised that some formatting such as charts, tables, and bullets is not supported in the web-based application and will be lost if you try to paste these items from a Word document. If you are having problems with pasted information not saving try the following: Save your Word Document using Save As and select Plain Text (*.txt) in the Save as type: field. Click on Save A file Conversion dialogue box should appear. Check the box labeled: Allow character substitution. Click on the OK button. Open this document in Notepad. Copy and paste into the appropriate text field. Q. What do I do if I created a renewal waiver instead of a new waiver? A. The system cannot transfer the information from one waiver type to another. To put the information in the correct application format, you will need to open up the renewal application in another browser and create a new application in another browser and copy from one to the other. Q. How do I enter an existing, approved waiver application into the web-based system? A. To enter existing applications follow the migration protocol that is located on the Home page under the Links & Downloads section on the left-side of the screen. Application Tools/Functions Q. What is the difference between Check and Validate? A. Validate allows you to perform a cross-appendix edit check of your waiver application either in its entirety or by individual appendix. Check, on the other hand, allows you to perform an edit check of the responses in a specific section or subsection only and does not include cross-checks with other sections or appendices. Q. How do I print sections of the waiver or the entire application? A. Click on the Print menu link. Select the Appendix you want to print or select the Entire Application, then click Create Printable View. This will open another browser window where you will select your browsers print function. When you are finished printing the application, close the print browser window. Q. Why doesnt the text in my applications text boxes print correctly when using Internet Explorer (IE) Version 7? A. The reason it does not seem to print correctly is because the print preview function in IE Version 7 defaults to the Shrink to Fit option and therefore does not print all of the text in the textboxes. To correct this: From your browser window select File and then Print Preview, or click on the down arrow on the printer icon in the IE 7 toolbar and select Print Preview The Print Preview menu defaults to the Shrink to Fit option:  Click on the drop-down menu and select 100%.  With the 100% option selected, the page(s) will recalculate and the preview will include the entire text for all of the textboxes. Amendments Q. When a state creates an amendment (to an approved application already in the data base) will the program automatically direct the state to exactly which sections to complete? A. No, but it will request that you summarize the changes that will be made. You will only need to make changes to the appropriate sections rather than completing the entire application again. Q. If a Microsoft Word version of a waiver has been approved, can it be amended in the web system? A. No. In order to amend a waiver application there must first be an approved application in the web system. Q. Is it possible to concurrently work on two or more draft amendments to the same waiver? A. Yes, but there are some issues that you need to consider. It is important to understand that amendments are not linked to each other prior to being approved by CMS.This is because the system cannot predict whether any given amendment will ever be submitted by the state or approved by CMS, so amendments exist separately until they are approved. This can lead to conflicts between the amendments, particularly when both amendments attempt to add the same piece of information (such as a service).Adding the same service to two draft concurrent amendments will lead to a duplicate service validation error, if you add a service to the second amendment that is also found in the first amendment. Other validation errors, related to the duplicate service error, may also appear an example would be missing Factor D information for a duplicate service in Appendix J. The timing of when the validation error appears will vary depending on submittal, approval, and effective dates of the amendments, but it will usually appear when the second amendment is submitted first.The duplicate service will have to be removed from the second amendment before it can be submitted to CMS. Please feel free to contact Thomson Reuters if you need additional information or assistance with this issue. User Roles and Assignments Q. Who controls state user access to the application? A. Access to a states waiver application is controlled by the State Sys/Op Manager and/or the Medicaid Director. Q. If I am a State Sys/Op Manager, how do I add new users to the system? A. Log-in to the system and click on Maintenance at the top of the finder screen and then select Edit Users. Scroll down to the end of the page and select Create New User. Enter in all the information requested and select Save. Q. Can I restrict certain state users to read only access? A. Yes. The State Sys/Op Manager can create a user to have a read-only role by selecting the state auxiliary role in the User Maintenance screen. Q. How do I assign state users edit rights to a specific waiver application? A. To assign state users edit rights to a specific waiver application, select the User Access button on Page One of the application, and check the desired boxes in the Access column. This will give users the ability to edit the application. Attachments Q. Is there a way to attach additional information or appendices to a waiver application? A. No, the application is self-contained and only information inputted in the existing fields is acceptable. The character limits in the text fields should be sufficient to convey necessary information without using attachments. Submit/Unsubmit Q. Can a waiver be submitted if it is not cost-neutral? A. No, because it will not pass the validation checks. Q. Once a state submits an application, is the content locked? A. Yes, unless CMS takes an action that unlocks it allowing the state to perform editing. Q. Who can submit the waiver application? A. Any individual assigned the State Medicaid Director role. Multiple Users Q. Can multiple users enter information into the application at the same time? Will they be locked out? A. Yes, multiple users can access an application and edit it at the same time. However, it is recommended that users coordinate their efforts and not edit the same appendices/sections to avoid overwriting each other's work. The last "save" is the one that counts. Version 3.4 vs. 3.5 Q. When will Version 3.4 of the web-based application no longer be available to users? A. Version 3.4 of the web-based application will be available until August 31, 2008. After that date, users will no longer have the option of working with 3.4 applications. Q. What will happen to existing Version 3.4 applications after August 31, 2008? A. Prior to August 31st, new amendments to existing waiver applications will continue to be created in the same version as the waiver application that is being amended. For example, if the application being amended is in Version 3.4, then the amendment will also be created in Version 3.4. Conversely, if the application being amended is in Version 3.5, then the amendment will also be created in Version 3.5. Since the launch of Version 3.5, all new and renewal waiver applications are automatically created in Version 3.5. After Aug 31st, all applications, whether new, renewals, or amendments, will automatically be created in Version 3.5. Once an amendment has been converted to Version 3.5 and approved by CMS, all other submitted but not yet approved Version 3.4 amendment applications for that waiver must be returned to the state for conversion to Version 3.5 and then re-submitted to CMS for approval.The new Version 3.5 information contained in the amendment application that first converted the waiver will be applied to subsequent amendment conversions. Q. Must the Quality Improvement modules in Version 3.5 be completed if a recently submitted application met the Version 3.4 quality improvement requirements? A. There is a temporary shortcut method to satisfy the validation requirements for the Quality Improvement modules in Version 3.5 applications that eliminates the need to enter detailed data. However, before the steps listed below are implemented, consult with your CMS representative first: Copy the 3.4 version of Appendix H and paste it into Main 8B in 3.5. In each of the quality areas throughout the appendices, complete as follows: a.i no need to enter performance measure information. a.ii input see 8a or 8a (note: any character will enable validation passage) b.i. enter characters as noted in a.ii above b.ii. check other box in chart and enter characters into text box (i.e,., 8a) timelines select no In Appendix H similar process a.i characters (ie., 8a) a. ii in App H check other, and enter characters (i.e., 8a) In both text boxes in b. enter characters. Spell Checking Q: Is there a way to spell check text entered into the on-line application? A: Some browsers, such as Firefox and Netscape, include spell check capability built into the browser. Others, such as Internet Explorer, do not. Q: Can spell check be added to Internet Explorer? A: Yes, there are add-ons and plug-ins available for download that will add spell check functionality to Internet Explorer. Many can be downloaded for free. Q: Where can I find these add-ons and/or plug-ins? A: You can use an internet search engine to obtain a list of available spell checkers for Internet Explorer. Try the search phrase Internet Explorer spell check. Q: Are these downloads safe? A: Any download carries the potential to also carry a virus. It is strongly recommended downloads be done from reputable sources and be scanned by up to date anti-virus software on your local pc before installing. Q: Do you recommend a particular spell checker? A: We do not endorse or recommend any specific spell check product. We do recommend that the user check with their IT support group for any guidance, recommendations, or restrictions prior to downloading or installing any software onto their pc. Q: Is there a way to spell check without using a browser plug-in? A: Yes there is, but it requires that the user spell check their text using a word processor (such as Microsoft Word) outside of the web-based waiver system. The user would copy the spell-checked text from the editor and paste it into the text box in their browser. Miscellaneous Q. Is there a place to put the old waiver number on the web-based application? A. Yes. There is a field in amendment, renewal, and migration applications to enter the old waiver number. Q. I received an unexpected error message, what should I do? A. Click on the underlined text report the problem when the unexpected error message appears, to send an email to the Thomson Reuters staff. Two ways to avoid unexpected error messages are 1) never use the browser back and forth arrows to navigate through the waiver application and 2) remain inactive on the waiver application for more than thirty minutes. Q. Can states delete applications that were inadvertently created? A. No, applications cannot be deleted at this time, but the expectation is that in the near future states will have the capability to hide draft applications not currently in use. Q. How will we be notified when our application is approved or disapproved? A. The State Medicaid Director and the State Sys/OP Manager roles will receive email notifications. Telecare Q. Can States elect to cover Telecare through HCBS Waivers? A. Yes, Through Section 1915(c) HCBS waivers, States have the opportunity to design specific service packages to meet the needs of individuals who would otherwise require an institutional level of care. At least one State has incorporated electronic monitoring as a method of delivering oversight, monitoring and cueing services to eligible individuals. Telecare offers the opportunity for individuals to receive needed reminders and monitoring while reducing the need for human assistance. While challenges exist to ensure that the use of technology in this regard respects the privacy and autonomy of individuals, it is an allowable method for delivering services under a 1915(c) waiver, so long as all concerns regarding privacy, choice, control, health and welfare and person-centered service plan development and implementation are adequately addressed. This method could be approved as a discrete service within a waiver, utilizing the "other" service category enumerated in 1915(c)(4)(B) of the Social Security Act. It could also be an approvable method for delivering services defined in the waiver that may be appropriate for the technology. In either situation, the State must describe reasonable provider qualifications, and safeguards, as well as the strategies they will employ to provide adequate oversight and quality improvement. CMS 372 Report Module Entering Information into the CMS 372 Report Q. How do I access the 372 Report Module? A. In the Waiver Finder screen select 372 Reports. A summary listing of your states 372 reports will display. Select detail to view a specific waivers 372 reports. Then select edit or browse as appropriate. Q. How do I create or add a new 372 report?* A. To add a new report to an existing waiver set of reports: At the bottom of your states listing of reports in the 372 Report Waiver Base Detail Finder, enter the waiver base number in the field provided. Select Add New Report for Selected Waiver Base. To add a report for a waiver for which no reports have been entered yet: At the bottom of your states listing of reports in the 372 Report Waiver Base Summary Finder, enter the waiver base number in the field provided. Select Add New Report. * Only state users can create and edit 372 reports. State users who do not have either the State Sys/OP Mgr. or State Medicaid Director roles in the system must be assigned edit access by one of these individuals. This is done by accessing the 372 report Maintenance screen and selecting the State User Access link and then designating individuals by checking the checkbox next to their names. While in the 372 report module, select the Maintenance link at the top of the screen. In the Maintenance Menu select the In Process Report link. Q. How do I sign up to receive the In Process Report via email? {CMS users only} A. Go to the 1915c maintenance screen. Select Report Distribution Maintenance. Select CMS 372 In Process Status Report from the drop-down menu. Designate (check) individuals to receive the report. When finished, click the back link to exit. Q. How should a State complete the quality section of the 372 in light of the Interim Procedural Guidance? A. The Interim Procedural Guidance (IPG) changed CMS quality review requirements, and hence the expectations on how States complete the 372 questions on quality. By its nature, the IPG was meant to be a transitional, temporary process ("interim"). It has always been CMS' plan to integrate the quality review with the 372 reporting process. Although we anticipate changes to the 372 report in the future, the state must report any changes to their approved Quality Improvement Strategy on the existing annual 372 report in the quality question areas of the form. In addition, states should report to CMS on their quality evidence by answering the questions on the 372, in line with their performance measures. The CMS quality review and reporting requirements are contained in the Quality letter #12. CMS expects that by early 2010, two changes will occur which will impact the annual waiver reporting process - 1) there will be a revised quality review process, which will address needed changes to the format of Interim Procedural Guidance, more closely aligning it with the Waiver Application; and 2) the CMS-64 expenditure reporting system will be changed to capture additional data on HCBS waivers. As states adopt Phase I of the revised CMS-64 system (MBESCBES), some information currently entered on the CMS-372 may also be entered in the 64.9 waiver feeder form (detail sheet) for each HCBS waiver, including a breakout of the costs for each statutory service, and capitated HCBS costs. For now, all "other" services will be recorded on an "other" line. In future phases of this work, CMS plans to capture expenditures for each and all approved waiver service. In addition, number of users of a service may now be entered on the 64.9 waiver sheet. As adoption of the revised 64 forms grow and as our revised quality review process is implemented, CMS will modify the reporting forms and requirements to reflect the new tools available. Q. What timeframes should be followed when submitting quality information on the 372s? A. The timeframes on the 372 are by waiver year, but the report is not due to be submitted to CMS until 18 months after the close of the waiver year. However, in order to maintain the most up to date information on the state's quality activities, the state should report quality information on the past 12 months. In other words the quality information will not coincide with the utilization and expenditure information on the report. If this has not been followed to date, the State may include this information on their next scheduled submission of the 372 form by reporting both on the previous waiver year, and up to the present with the addition of information.
